With surgical and immunosuppressive advances in thoracic organ transplantation over the past 3 decades and attendant improved long-term outcomes, post-transplant pregnancy is an achievable goal for many lung transplant (LT) and heart transplant (HT) recipients. The keys to successful post-transplant pregnancy are preconception and contraceptive planning, appropriate patient risk assessment with comprehensive risk stratification, and optimization of maternal comorbidities and fetal health through careful monitoring... The purpose of this statement is to summarize the current evidence and provide guidance surrounding preconception counseling, patient risk assessment, and medical management, maternal and fetal outcomes, obstetric management, and pharmacologic considerations.
